Parent Registration Agreement

Liability Waiver & Release

I, the undersigned parent/legal guardian, hereby acknowledge that my child's participation inSparkMinds camp programs (operated by TwinAI Inc., Belmont, MA) involves activities that carry inherent risks, including but not limited to: use of electronic equipment, exposure to online content, and physical activities during breaks.

I voluntarily assume all risks of injury or illness to my child and agree to release, indemnify, and hold harmless TwinAI Inc., its officers, directors, employees, instructors, teaching assistants, and agents from any and all claims, liabilities, damages, or expenses arising from my child's participation in camp activities, except in cases of gross negligence or willful misconduct.

Medical Authorization

I certify that the medical information provided during registration is accurate and complete. I authorize SparkMinds staff to administer basic first aid and, in the event of a medical emergency, to seek emergency medical treatment for my child. I understand that I will be contacted as soon as possible in any medical situation.

I agree to inform SparkMinds of any changes to my child's medical conditions, medications, or allergies prior to or during the camp session.

Photo & Video Release

I grant SparkMinds and TwinAI Inc. permission to photograph, video record, and/or audio record my child during camp activities. I understand these materials may be used for:

• Marketing and promotional materials (website, social media, flyers)
• Internal training and quality improvement
• Documentation of camp activities and student projects

I understand I may opt out of this release during registration by unchecking the photo release option. If I opt out, SparkMinds will make reasonable efforts to exclude my child from photographs and videos used for promotional purposes.

Technology & AI Use Consent

I understand and agree that:

• My child will use AI tools (such as ChatGPT, Claude, image generators, and coding tools) as part of the curriculum.
• While SparkMinds implements age-appropriate content filters and supervision, AI tools may occasionally generate unexpected content.
• My child will be supervised during all AI tool usage.
• My child's interactions with AI tools may be logged for safety monitoring purposes.
• My child will be taught responsible and ethical AI usage.

Code of Conduct Acknowledgment

I have reviewed and agree to the Student Code of Conduct. I understand that:

• My child is expected to behave respectfully toward staff, instructors, and other students.
• Bullying, harassment, or violent behavior will not be tolerated.
• Misuse of technology or AI tools (generating inappropriate content, bypassing filters) may result in dismissal.
• SparkMinds reserves the right to dismiss my child without refund for serious or repeated behavioral violations.
• I will be contacted before any dismissal decision is made, except in cases posing immediate safety concerns.

Drop-off & Pick-up Authorization

• I understand that camp hours are 9:00 AM to 3:00 PM, Monday through Friday.
• Drop-off begins at 8:45 AM.
• Pick-up must be completed by 3:15 PM. A late fee of $1 per minute applies after 3:15 PM.
• Only authorized persons listed during registration may pick up my child.
• Photo ID will be required for pick-up verification.
• I will notify SparkMinds in advance if someone not on the authorized list will be picking up my child.

Refund Policy Acknowledgment

I have reviewed and agree to the Refund & Cancellation Policy.

Privacy Policy Acknowledgment

I have reviewed and agree to the Privacy Policy, including the collection and use of my child's information as described therein.
